Important Information for New Patients

Welcome to Family Foot Care & Surgery, LLC! We’re glad you’ve chosen to entrust Dr. Sanjay Patel and our team with your foot health. Taking the plunge by making an appointment for your foot issues is a big, and important, first step toward healthier feet. We’re looking forward to getting to know you and your family, and to walk with you toward better, comfier, and healthier feet.

Your First Appointment

We want to make you as comfortable as possible for your first appointment, and make sure you have plenty of time to spend with the doctor, getting your questions answered. To make sure your check-in and appointment go as smoothly as possible, it’s best to prepare for them ahead of time. Collect your insurance cards and a photo ID and bring them with you to check in. We’ll need them to be able to bill your insurance. To speed up the check-in process, we’ve also made it possible to fill out the required paperwork ahead of time and bring it with you. Our new patient registration forms and HIPAA agreement can be found below so you can print them off and fill them out beforehand.

For your medical history, we may need a list of current medications you are taking as well as relevant past injuries or procedures. It’s also a good idea to write down your foot or ankle symptoms and any questions you may have to make sure you don’t forget anything important. That way, everything gets addressed during your appointment. We also recommend bringing any orthotics you use, shoes you normally wear, and athletic shoes so our team can get a clearer picture of your gait and potential foot needs.

To Know Before You Go

We partner with most insurance providers and we accept cash if you plan to pay out-of-pocket. For insurance, you will need to cover any copays or deductible fees at the time of service, so please come prepared. If you are not sure if we partner with your insurance provider, contact us before your appointment to ask.

If anything happens and you won’t be able to make your appointment, we ask that you cancel or reschedule at least 24 hours in advance.
